Mental health is a topic that's often hard to talk about, and unfortunately not without controversy. A forthcoming edition of an influential U.S. psychiatric manual, known as the American Psychiatric Association's Diagnostic and Statistical Manual (DSM-5) due out in May 2013 is expected to increase the numbers of people in the general population that could be diagnosed with a mental illness. Yet some experts argue that what patients need is help and understanding, not a new label or a bottle of prescription medication.
Grief, for example can be profound and long lasting after the loss of a loved one, yet the new guidelines now classify it as "major depressive disorder". Anxiety in social situations and other areas of life in today's demanding, unforgiving world is understandable - yet everyday worries didn't rise to this level, until now. These feelings are human truths that we as a society need to accept and try to assist, not regard as symptoms of some underlying mental illness according to Peter Kinderman, a professor of clinical psychology and head of the Institute of Psychology, Health and Society at the University of Liverpool.
Under the new guidelines, childhood temper tantrums will now be labeled "disruptive mood dysregulation disorder", the threshold for "generalized anxiety disorder" will be much more relaxed, so that struggling with the worries of daily life are now classified as part of a "mental illness". Even some less pleasant human behaviors will now be considered mental illnesses. Overeating becomes "binge eating disorder" and addictions will widen to include such disorders as "internet addiction" and "sex addiction".
There are some, like Kinderman, who believe that psychiatric diagnoses are harmful and perhaps even scienti8fically invalid. Yet today a diagnosis is necessary to access services, though it can also be the source of misuse or overuse of mediations that bring the risks of serious side effects. What's more, the use of the language of illness suggests that the roots of these issues come from abnormalities in the brain or biology, often termed "chemical imbalances". But this thinking shuts off the idea that there are social and psychological issues that may also be behind our distress.
Too often, the "solution" is to prescribe medication, even though antidepressant or antipsychotic drugs have some significant side effects, and evidence that these medications work is hardly overwhelming. In fact, recent findings suggest that distressing experiences come from complicated interactions between biological, social and psychological factors. Abuse, poverty, social exclusion also play a part.

Perhaps it's time to change the approach, update it for the 21st century. With the economy as it is, is it any wonder our mental health is impacted... making us feel miserable, hopeless and suffer lack of confidence. Such problems call for help and support, they may not call for medications, to be described as an illness or disorder. Social and psychological, not medical solutions are what's needed here. Prevention, early intervention, reducing inequalities and facing down stigma all have a part in addressing mental health issues.
Having hard work, time and effort go into DSM-5 and the publication is certainly of value, but equally important is the thinking that other, tougher changes must be made to address the issues and help people reach their potential.
